The proficiency testing for determination of pesticides residues in mango pulp was the third work of the partnership established by INMETRO and INCQS/Fiocruz. Three mango pulp samples were sent to each participant laboratory, two being spiked with the pesticides and one exempt of pesticides. The added pesticides were: deltamethrin, ethion, fenitrothion, malathion, and permethrin. The evaluation of the results of the homogeneity and the stability tests, as well as the determination of the assigned value was made in agreement with ISO GUIDE 35 and ISO 13528, assuming the samples were considered homogeneous and stable for the studied period. The assigned values and the standard deviation for proficiency evaluation was calculated using the robust algorithm, according to ISO 13528, and the evaluation of the results was carried through in accordance with ABNT ISO/IEC Guide 43-1. The z-score graphs and confidence ellipse was also used in the evaluation of the results. In the evaluation carried through from the values of the z-scores, 71% of the reported results were considered satisfactory based on the results found for this index. The evaluation of the analytical viability for the determination of each pesticide and of the analytical capacity of the participant laboratories was carried through. A summarized view of the chromatographic techniques and of preparation of sample used by the participant laboratories was also carried through in this work.

In a previous work we proposed a categorial structure for the representation of adverse drug reactions consisting of 16 semantic categories and 20 relations. We present an implementation of this categorial structure in Protégé based on four WHO-ART system organ classes: Gastro-intestinal system disorders, Liver and biliary system disorders, Central & peripheric nervous system disorders, and Psychiatric disorders. We compared classification according to anatomy using SNOMED CT within the PharmARTS tool and the FMA with the Pellet reasoner. This ontology contains 210 concepts for Gastroenterology, 66 concepts for Psychiatry and 85 concepts for Neurology. Classification of disorders located in the upper gastro intestinal tract was similar using both SNOMED CT and the FMA. This work is a first step towards the comparison of two models of anatomy within a common ontology of adverse drug reactions.

Semantic interoperability (SIOp) is a major issue for health care systems having to share information across professionals, teams, legacies, countries, languages and citizens. The World Health Organisation (WHO) develops and updates a family of health care terminologies (ICD, ICF, ICHI and ICPS) and has embarked on an open web-based cooperation to revise ICD 11 using ontology driven tools. The International Health Terminology Standard Development Organisation (IHTSDO) updates, translates and maps SNOMED CT to ICD 10. We present the application of the CEN/ISO standard on categorial structure to bind terminologies and ontologies to harmonise and to map between these international terminologies.

International cross-border private hospital chains need to apply the standards for foreign currency translation in order to consolidate the balance sheet and income statements. This not only exposes such chains to exchange rate fluctuations in different ways, but also creates added requirements for enterprise-level IT systems especially when they produce parameters which are used to measure the financial and operational performance of the foreign subsidiary or the parent hospital. Such systems would need to come to terms with the complexities involved in such currency-related translations in order to provide the correct data for performance benchmarking.

The chemotherapy of the metastatic breast cancer is characterized by the diversity of the treatment protocols and the utilisation of new expensive molecules posing the double problem of outcomes for the patients and financial effects for the hospitals. This survey describes the different chemotherapy treatments prescribed in the metastatic breast cancer and the direct costs supported by the hospitals according to the patient survival time. A cohort of 371 patients treated for a metastatic breast cancer was followed in three hospitals of the Rhone-Alpes region between 2001 and 2006. The detail of their different antineoplasic treatments, as well as the purchase cost of the drugs and their cost of hospital administration, the cost of the other hospital stays are presented in relation with the survival. The median survival time (35,8 months; CI 95%:[31.7-39.1]) since the first metastasis does not differ significantly according to the hospital. Ninety-three different chemotherapy protocols are observed combining from one to five molecules. Thirty-two different molecules are identified. In first line treatment, there is a significant difference in the use of the new molecules according to hospital (Chi(2) test; P < 10(-3)). The average cost of a chemotherapy treatment is 3,919 euro (+/- 8,069 euro), the higher cost is observed for trastuzumab (23,443 euro). The average time period before the beginning of a new chemotherapy line is 212 days (+/- 237 days) and the mean cost of hospital stay during this period is 3,903 euro (+/- 4,097 euro). If no impact of the chemotherapy treatment strategy is observed on the survival time of the patient, it is the opposite for the hospital treatment cost. These results are asking for a better control system of the authorization procedure of new molecules marketing and the harmonization of the practices.

Ultraluminous X-ray sources are extragalactic objects located outside the nucleus of the host galaxy with bolometric luminosities exceeding 10(39) erg s(-1). These extreme luminosities-if the emission is isotropic and below the theoretical (Eddington) limit, where the radiation pressure is balanced by the gravitational pressure-imply the presence of an accreting black hole with a mass of approximately 10(2)-10(5) solar masses (). The existence of such intermediate-mass black holes is in dispute, and though many candidates have been proposed, none are widely accepted as definitive. Here we report the detection of a variable X-ray source with a maximum 0.2-10 keV luminosity of up to 1.1 x 10(42) erg s(-1) in the edge-on spiral galaxy ESO 243-49, with an implied conservative lower limit for the mass of the black hole of approximately 500.

A new French coding system of clinical procedures, the Classification Commune Des Actes Medicaux (CCAM), has been developed at the turn of the millennium (between 1996 and 2001). Two methodologies were used: a traditional domain-experts consensus method, and an artificial-intelligence-based semantic representation. An economic evaluation of clinical procedures was also undertaken for the rating for fee-for-service payment. We present the methodologies used and stress how the European Union research project,'European Consortium, Generalised Architecture for Languages, Encyclopaedias and Nomenclatures in Medicine'(GALEN), facilitated the sharing and maintaining of consistent medical knowledge. This country case study highlights the significant cost to individual countries in developing their own classifications in isolation. It also demonstrates the benefits of contributing to international efforts such as GALEN that enable harmonisation, yet still allow for diversity.

WHO-ART and MedDRA are the terminologies used in pharmacovigilance for coding of adverse drug reactions and statistical analysis. In previous work we showed that tools for automated signal detection and access to pharmacovigilance databases would benefit from terminological reasoning in order to provide improved groupings of terms describing the same medical condition. Such reasoning depends on formal definitions that are absent in both terminologies. A Categorial structure is defined as a minimal set of health care domain constraints which represents a biomedical terminology in a precise healthcare domain. Here we present a draft for a lite ontological model consisting in 19 semantic categories and 16 relations for the representation of adverse drug reactions. From this model we selected 8 semantic categories for the categorial structure. This study was restricted to WHO-ART and additional research is required in order to provide complete coverage of MedDRA.

The influence of surface growth (two-dimensional microcarriers) and three-dimensional growth (aggregates and macroporous supports) in agitated, suspended batch culture systems upon growth and productivity of BHK was compared. Cultures using three porous microcarriers (CultiSpher G, Cellsnow EX, and Cytocell), one nonporous microcarrier (Cytodex 3) and natural aggregates were performed in stirred tanks using two different agitation rates (60 and 100 RPM). With the exception of Cytocell, cell growth, viability, and productivity were similar when three-dimensional structures (porous microcarriers and aggregates) were used. Nonporous microcarriers only compared well at 60 RPM as growth ceased under overagitation. These results suggest that cultures less susceptible to fluid shear are advantageous for scale-up.(c) 1996 John Wiley & Sons, Inc.

The goal of the present study was to assess whether in vitro-differentiated N1E-115 cells supported by a collagen membrane would enhance rat sciatic nerve regeneration after a crush injury. To set up an appropriate experimental model for investigating the effects of neural cell transplantation, we have recently described the sequence of functional and morphologic changes occurring after a standardized sciatic nerve crush injury with a nonserrated clamp. Functional recovery was evaluated using the sciatic functional index, the static sciatic index, the extensor postural thrust, the withdrawal reflex latency, and ankle kinematics. In addition, histomorphometric analysis was carried out on regenerated nerve fibers by means of the 2D-disector method. Based on the results of the EPT and of some of the ankle locomotor kinematic parameters analyzed, the hypothesis that N1E-115 cells may enhance nerve regeneration is partially supported although histomorphometry disclosed no significant difference in nerve fiber regeneration between the different experimental groups. Therefore, results suggest that enrichment of equine type III collagen membrane with the N1E-115 cellular system in the rat sciatic nerve crush model may support recovery, at least in terms of motor function. The discrepancy between functional and morphological results also suggests that the combined use of functional and morphological analysis should be recommended for an overall assessment of recovery in nerve regeneration studies.(c) 2008 Wiley-Liss, Inc.
