A search for a Higgs boson in the four-lepton decay channel H→ZZ, with each Z boson decaying to an electron or muon pair, is reported. The search covers Higgs boson mass hypotheses in the range of 110<m_{H}<600  GeV. The analysis uses data corresponding to an integrated luminosity of 4.7  fb^{-1} recorded by the CMS detector in pp collisions at sqrt[s]=7  TeV from the LHC. Seventy-two events are observed with four-lepton invariant mass m_{4ℓ}>100  GeV (with 13 below 160 GeV), while 67.1±6.0 (9.5±1.3) events are expected from background. The four-lepton mass distribution is consistent with the expectation of standard model background production of ZZ pairs. Upper limits at 95% confidence level exclude the standard model Higgs boson in the ranges of 134-158 GeV, 180-305 GeV, and 340-465 GeV. Small excesses of events are observed around masses of 119, 126, and 320 GeV, making the observed limits weaker than expected in the absence of a signal.

A search for signatures of extra spatial dimensions in the diphoton invariant-mass spectrum has been performed with the CMS detector at the LHC. No excess of events above the standard model expectation is observed using a data sample collected in proton-proton collisions at sqrt[s]=7  TeV corresponding to an integrated luminosity of 2.2  fb^{-1}. In the context of the large-extra-dimensions model, lower limits are set on the effective Planck scale in the range of 2.3-3.8 TeV at the 95% confidence level. These limits are the most restrictive bounds on virtual-graviton exchange to date. The most restrictive lower limits to date are also set on the mass of the first graviton excitation in the Randall-Sundrum model in the range of 0.86-1.84 TeV, for values of the associated coupling parameter between 0.01 and 0.10.

Aims: A major problem in cancer pharmacology is the unpredictability of the outcome of therapy, both in terms of tumor response and host toxicity. Pharmacogenetic variability associated with the drug metabolizing enzyme systems is a major determinant of variations in these outcomes. Materials and Methods: A case-control study of 100 male cases of head and neck squamous cell carcinoma and equal number of healthy controls was conducted. Genomic DNA isolated from blood samples collected from controls and patients was studied by PCR-RFLP technique for CYP2D6 polymorphism. All patients received three cycles of cisplatinum-based sequential chemoradiotherapy. Results: The increased frequency of variant genotypes was associated with a statistically significant increase in the risk in the cases both in CYP2D6FNx014 and FNx0110. The effect of interaction of the risk modifiers such as cigarette smoking or tobacco chewing or alcohol drinking with the CYP2D6 genotypes in the controls and patients was found to be significant. Response to therapy in patients with variant genotypes of CYP2D6 (CYP2D6FNx014 and CYP2D6FNx0110) and treated with radio and chemotherapy regimen was poor. Conclusions: Functional enzyme deficiencies due to polymorphism in CYPs are not only important in enhancing susceptibility to head and neck squamous cell carcinoma but also in determining chemotherapeutic response.

The present investigation was undertaken to study the role of working conditions in occupational hazards among bidi rollers occupationally exposed to tobacco dust with reference to DNA damage in peripheral blood lymphocytes. Initially, biomonitoring was conducted by estimating urinary thioether to detect the extent of xenobiotic exposure, and genotoxicity was evaluated by assessing DNA damage and micronucleus frequency in buccal epithelial cells in female bidi rollers occupationally exposed to bidi tobacco dust. Student's t-test was used to test the significance between the means. Results showed a significant increase in urinary thioether level in during-shift urine samples as compared to pre-shift samples, which indicated exposure of bidi rollers to xenobiotic compounds. A significant increase in DNA damage was observed in the rollers working in confined environment as compared to those who worked in open and mixed kind of working conditions. Keeping in view the adverse effects of tobacco inhalation on the genotoxic effects in bidi rollers as an occupational hazard and in order to minimize the hazardous effects, it is recommended that masks should be worn by the bidi rollers during work to minimize inhalation of tobacco dust. Gloves should be worn particularly if there are bruises etc. in the palm. To minimize the absorption through eyes, covered glasses should be worn. The entire process of bidi rolling may be done preferably under well ventilated conditions. Due care to be taken to sit in the direction facing the direction of wind to avoid inhalation of blown away tobacco dust.

The random-field Ising model of hysteresis is generalized to dilute magnets and is solved on a Bethe lattice. Exact expressions for the major and minor hysteresis loops are obtained. In the strongly dilute limit the model provides a simple and useful understanding of the shapes of hysteresis loops in magnetic rock samples.

An infectious cDNA clone of a genotype 3 strain of hepatitis E virus adapted to growth in HepG2/C3A human hepatoma cells was constructed. This virus was unusual in that the hypervariable region of the adapted virus contained a 171 nucleotide insertion that encoded 58 amino acids of human S17 ribosomal protein. Analyses of virus from 6 serial passages indicated that genomes with this insert, although initially rare, were selected during the first passage, suggesting it conferred a significant growth advantage. RNA transcripts from this cDNA and the viruses encoded by them were infectious for cells of both human and swine origin, the major host species for this zoonotic virus. Mutagenesis studies demonstrated that the S17 insert was a major factor in cell culture adaptation. Introduction of 54 synonomous mutations into the insert had no detectable effect, thus implicating protein, rather than RNA, as the important component. Truncation of the insert by 50% decreased levels of successful transfection approximately 3-fold. Substitution of the S17 sequence by a different ribosomal protein sequence or by GTPase activating protein sequence resulted in a partial enhancement of transfection levels whereas substitution with 58 amino acids of green fluoresent protein had no effect. Therefore, both sequence length and amino acid composition of the insert were important. The S17 sequence did not affect transfection of human hepatoma cells when inserted into the hypervariable region of a genotype 1 strain but this chimeric genome acquired a dramatic ability to replicate in hamster cells.

Abstract Background: THUNDERBEAT™ (TB)(Olympus, Japan) simultaneously delivers ultrasonically generated frictional heat energy and electrically generated bipolar energy. The aim of this study was to evaluate the versatility, bursting pressure, thermal spread, and dissection time of the TB compared with commercially available devices: Harmonic(®) ACE (HA)(Ethicon Endo-Surgery, USA), LigaSure™ V (LIG)(Covidien, USA), and EnSeal(®)(Ethicon). Methods: An acute study was done with 10 female Yorkshire pigs (weighing 30-35 kg). Samples 2 cm long of small (2-3 mm)-, medium (4-5 mm)-, and large (6-7 mm)-diameter vessels were created. One end of the sample was sent for histological evaluation, and the other was used for burst pressure testing in a blinded fashion. Versatility was defined as the performance of the surgical instrument based on the following five variables, using a score from 1 to 5 (1=worst, 5=best), adjusted by coefficient of variable importance with weighted distribution: hemostasis, 0.275; histologic sealing, 0.275; cutting, 0.2; dissection, 0.15; and tissue manipulation, 0.1. There were 80 trials per vessel group and 60 trials per instrument group, giving a total of 240 samples. Results: Versatility score was higher (P<.01) and dissection time was shorter (P<.01) using TB compared with the other three devices. Bursting pressure was similar among TB and the other three instruments. Thermal spread at surgery was similar between TB and HA (P=.4167), TB and EnSeal (P=.6817), and TB and LIG (P=.8254). Difference in thermal spread was noted between EnSeal and HA (P=.0087) and HA and LIG (P=.0167). Conclusion: TB has a higher versatility compared with the other instruments tested with faster dissection speed, similar bursting pressure, and acceptable thermal spread. This new energy device is an appealing, safe alternative for cutting, coagulation, and tissue dissection during surgery and should decrease time and increase versatility during surgical procedures.

PURPOSE: To evaluate whether information from the initial fractions can determine which patients are likely to consistently exceed their planning dose-volume constraints during the course of radiotherapy for prostate cancer. METHODS AND MATERIALS: Ten patients with high-risk prostate cancer were treated with helical tomotherapy to a dose of 60 Gy in 20 fractions. The prostate, rectum, and bladder were recontoured on their daily megavoltage computed tomography scans and the dose was recalculated. The bladder and rectal volumes (in mL) receiving ≥100% and ≥70% of the prescribed dose in each fraction and in the original plans were recorded. A fraction for which the difference between planned and delivered was more than 2 mL was considered a volume failure. Similarly if the difference in the planned and delivered maximum dose (D(max)) was ≥1% for the rectum and bladder, the fraction was considered a dose failure. Each patient's first 3 to 5 fractions were analyzed to determine if they correctly identified those patients who would consistently fail (i.e., ≥20% of fractions) during the course of their radiotherapy. RESULTS: Six parameters were studied; the rectal volume (RV) and bladder volumes (BV)(in mL) received ≥100% and ≥70% of the prescribed dose and maximum dose to 2 mL of the rectum and bladder. This was given by RV(100), RV(70), BV(100), BV(70), RD(max), and BD(max), respectively. When more than 1 of the first 3 fractions exceed the planning constraint as defined, it accurately predicts consistent failures through the course of the treatment. This method is able to correctly identify the consistent failures about 80%(RV(70), BV(100), and RV(100)), 90%(BV(70)), and 100%(RD(max) and BD(max)) of the times. CONCLUSIONS: This study demonstrates the feasibility of a method accurately identifying patients who are likely to consistently exceed the planning constraints during the course of their treatment, using information from the first 3 to 5 fractions.

PURPOSE To evaluate three-dimensional needle displacements during multifractionated interstitial brachytherapy (BT) for cervical cancers. METHODS AND MATERIALS Patients scheduled to undergo pelvic interstitial BT for postoperative and or postradiation vault recurrences were included from November 2009 to December 2010. All procedures were performed under spinal anesthesia. Postprocedure BT planning CT scans were obtained with patients in supine position with arms on the chest (interslice thickness of 3 mm). Thereafter, verification CT was repeated at every alternate fraction. Needle displacements were measured in reference to a relocatable bony point. The mean cranial, caudal, anteroposterior, and mediolateral displacements were recorded. Statistical significance of mean interfraction displacements was evaluated with Wilcoxon Test. RESULTS Twenty patients were included. Seventeen received boost BT (20 Gy/5 fractions/3 days) after external radiation, three received radical BT alone (36 Gy/9 fractions/5-8 days). An average of three scans (range, 2-3) were available per patient, and 357 needle displacements were analyzed. For the entire study cohort, the average of mean needle displacement was 2.5 mm (range, 0-7.4), 17.4 mm (range, 0-27.9), 1.7 mm (range, 0-6.7), 2.1 mm (range, 0-9.5), 1.7 mm (range, 0-9.3), and 0.6 mm (range, 0-7.8) in cranial, caudal, anterior, posterior, right, and left directions, respectively. The mean displacement in the caudal direction was higher between Days 1 and 2 than that between Days 2 and 3 (13.4 mm vs. 3.8 mm; p = 0.01). The average caudal displacements were no different between reirradiation and boost cohort (15.2 vs. 17.8 mm). CONCLUSIONS Clinically significant caudal displacements occur during multifractionated pelvic brachytherapy. Optimal margins need to be incorporated while preplanning brachytherapy to account for interfraction displacements.
